Group Enrollment Instructions - 2+ lives

Voluntary is for the following situations:

  • Available to individuals and groups 2+

  • No (or minimal) contribution by employer

  • 1099 employees

  • Associations

  • Online or paper applications

 

Employer Contribution will lower rates and allows extra benefits  like maternity. 

  • Access to discounted rates

  • Access to Maternity benefit

  • Able to remove pre-existing limitation clause

  • Available to groups 25+

  • Increased participation when employer contributes

Each voluntary group needs to complete the following:

  • New Case Submission Form

  • Each participant needs to complete the online e-application or paper application.

    • AUIC enrollment form ($2 monthly fee)

    • Core Health enrollment form

  • If employer would like to payroll deduct then complete list bill form.  The participants can be billed directly also (no list bill form needed).

Each employer group must complete the following:

  • New Case Submission Form

    • Effective date

    • Eligible employee classes (plans)

    • More detailed information regarding employer

    • Contribution percentage

  • Each participant needs to complete the enrollment form

    • Paper application

    • E-application (Coming Soon!)

    • Waivers or employees not participating

  • 1st months premium
